Can you eat spaghetti and meat sauce on a ketogenic diet? Not if you use regular spaghetti with a tomato sauce that’s high in added sugar.
So how can you quench cravings for your favorite comfort food without completely blowing your keto diet? With this keto friendly spaghetti sauce recipe, of course. This is one of my favorite recipes when it comes to sticking to a low-carb diet.
Creamy spicy spaghetti sauce is a great way to curb cravings for Italian style cooking, without sabotaging your weight loss efforts.

Creamy Spicy Spaghetti Sauce Ingredients
The ingredients list for this low carb spaghetti sauce meal are pretty basic since keeping keto meals simple helps make it simple to stick with keto.
The base of this low-carb pasta sauce is made up of ground beef and a low sugar marinara sauce. Since many of the regular pasta sauces in grocery stores have too much sugar, I order Primal Kitchen pasta sauce. Rao’s is another one of my favorite low carb sauces that you can find in the store.
Use High Quality Ingredients
Brands like Primal Kitchen and Rao’s are great options not since they have fewer carbs than higher carb alternatives, but they’re also a great choice since there’s no highly processed vegetable oils in the ingredients list. Instead of using cheaper oils like canola or soybean oil, these brands use olive oil in their sauces. This can make a big difference when it comes to weight loss on a keto diet.
I also prefer to use grass-fed beef whenever possible since conventional meat contains more toxins and less nutrients. If it’s difficult to source high quality beef near you, check out Butcher Box. They deliver grass-fed, organic meat right to your doorstep.
Add Some Healthy Fats
This keto spaghetti recipe adds two kinds of fat to the sauce: heavy whipping cream and butter. Not only do these add a creamy flavor, but getting enough healthy fats on a keto diet is important. Those who preach low calorie keto aren’t doing you any favors. Your body needs to feel safe with enough nutrients coming in in order to let go of stored fat.
I’m not saying to eat all of the fat, since that won’t work either – but it’s important to find the right balance for your body. Keeping grams of carbs low is important on keto … but in many cases, eating enough fat can be equally as important. This is especially the case if you’ve been part of the yo-yo diet world for sometime.
Veggies & Spices
Since nutrient dense foods are also important on keto, I added some of my favorite chopped vegetables into the sauce. This recipe calls for diced onions and peppers, but feel free to add any of your favorites. Chopped zucchini is another great option I sometimes add.
Rounding out the keto friendly spaghetti sauce ingredients list are the spices. I love this recipe with minced garlic, crushed red pepper to add spice, and Italian seasoning. OK – let’s face it … I love pretty much every recipe with garlic.
Feel free to mix up the flavor in this sauce by adding fresh basil, black pepper or any other spice you love in your low carb dishes.
How to Make Creamy Spicy Spaghetti Sauce
The first step is to create your homemade keto spaghetti sauce. Of course, you’re using a jar sauce as a base, so it’s not completely homemade … but you will zhush it up with spices and healthy fats. If you’re a skilled cook, by all means make your homemade sauce from scratch. Not all of us have those types of culinary skills.
Start by melting 2 tablespoons of butter in a saucepan over medium-heat. As the butter melts, stir in the minced garlic. Allow it to lightly brown. Then begin adding the heavy cream into the saucepan.
Add in 1/2 a cup of heavy cream and stir. Allow the sauce to thicken a little before adding the additional 1/2 cup of heavy cream. Stir frequently to allow the sauce to thicken. Stir on occasion. Allow the sauce to thicken for 5-10 minutes each time after adding the cream. Then pour in the marinara sauce. Stir well.
The Ground Beef
The second step in this easy keto spaghetti sauce recipe is to brown the beef. You can do this while the sauce is cooking.
Brown the beef in a large skillet over medium-high heat. As it browns, stir in a teaspoon of minced garlic. Sprinkle with Italian seasoning and add crushed red pepper to taste. Remember a little goes a long way. You can always add more later if you decide you want a little more spice in the sauce.
Another option for this sauce is to replace ground beef with ground Italian sausage instead. Both varieties make for a delicious sauce.
Once the spices are mixed into the meat, you can add in the chopped veggies. Add them sooner if you prefer them soft or towards the end if you like crunchy vegetables.
Combine the Meat and Sauce and Serve
Once the meat is fully cooked, remove from heat and pour the sauce over the meat. Stir with a wooden spoon to combine well. You can sprinkle with parmesan cheese if you want to add even more flavor to this recipe.
The best part of this keto spaghetti recipe is you can serve it as is as an all-in-one meal. This is how I made the leap from a standard American diet to a low carb lifestyle. I previously used this sauce on top of traditional spaghetti. When I wanted to lower carbs, I ate less noodles. I continued to remove the noodles until I was just eating this sauce by itself, almost like a chili.
If you miss spaghetti noodles, there are quite a few good substitutes when it comes to low-carb noodles. Both zucchini noodles and spaghetti squash are great options. Basically, any type of vegetable noodles could make a good addition to this recipe.
Some swear by miracle noodles, but I can’t recommend them only because I haven’t tried them. You could even make your own keto noodles out of almond flour and egg yolks. There are plenty of keto-friendly noodles recipes available on Pinterest.

Pasta lovers can rejoice, even on a low carb diet, since this keto spaghetti recipe made with low carb pasta sauce is the perfect replacement for regular spaghetti that’s too high in carbs. With a little sweetness and a little spice, it’s the perfect option when you’re craving keto pastas.
